Sports Physicals - July 29th at Medina School
All students participating in sports for the 2022-2023 school year must have a valid sports physical. Trisha Fennern, Nurse Practitioner, will be conducting sports physicals on Friday, July 29 at Medina School from 8:30 AM until 3:00 PM. Please contact Teresa at Medina School (701-486-3121) by July 22 to set up an appointment. The cost of the physicals is $50. Physical forms are available at the school or they may be accessed on the NDHSAA website. Please bring these completed forms with you the day of your physical. Physicals will not be completed unless parental forms are signed, and payment is made. Please arrive 10 minutes prior to your appointment to complete your paperwork. Payment can be made by cash, credit card, or check. If you have any questions, please call the school.
